Order Forms are available on the login screen. Please print several and have them on hand for your next order.

Shipping Labels
After you register, you will receive pre-paid shipping labels. When you are running low on labels, please check the appropriate box on one of your laboratory order forms, or login to your account and click the Request Printed Labels link.

Packing Instructions
(Failure to follow these instructions may result in an additional $5 handling fee)

Take your impression...	and wrap the impression in bubble wrap. Secure the bubble wrap with rubber bands.
Place the wrapped impression in a Zip-Lock™ bag.	• The impression must be wrapped in appropriate padding. • DO NOT place the order form in the Zip-Lock bag. • The Laboratory Order Form must be attached to the Zip-Lock bag.
Keep a copy of the Laboratory Order Form for your records. Don't forget to attach the Laboratory Order Form to the Zip-Lock bag. Use a stapler or wrap the Laboratory Order Form around the Zip-Lock bag and secure it with a rubber band. Place the sealed Zip-Lock bag containing the wrapped impression and the attached Laboratory Order Form into a shipping box. Tape the box closed with strong packing tape. Affix the pre-paid return service label on the box. Please make sure all of the barcodes are flat and easily scanned to avoid delays in shipping.
